Make a Referral
We respond to all referral enquiries within 4 working hours. Our referrals line is monitored 24 hours a day, 7 days a week - including evenings, weekends and bank holidays.
How referrals work
We've designed our referral process to be straightforward and responsive. Here's what to expect.
-
Initial contact
Complete the secure referral form below or email us directly to check availability.
-
Matching discussion
Our Registered Manager will speak with you directly to understand the young person's needs, current situation, and what a successful placement looks like. We'll be honest if we don't think we're the right fit.
-
Documentation review
We'll ask you to share relevant documentation so we can carry out a thorough matching assessment. The checklist below sets out what's essential and what's helpful to have.
-
Decision & placement planning
Once we've agreed a match, we'll work with you to plan the admission, prepare the young person, and arrange an introductory visit where appropriate. We aim to move quickly without cutting corners.
What we'll need
To assess whether we can offer a safe and suitable placement, we need certain information about the young person. Please share as much as you can at the referral stage.
Essential
- Placement request / referral form
- Care plan (or draft care plan)
- Risk assessment
- Education, Health and Care Plan (if applicable)
- Previous placement history
- Chronology
- Court orders / legal status
Helpful to have
- Previous placement summaries or reports
- Health passport / medical history
- Educational records and current school contact
- CAMHS assessment or referral letters
- Relevant police intelligence (where shareable)
- The young person's views on placement
- Family contact arrangements
Missing documentation should not prevent you from making an initial referral. Call us first and we can talk through what you have.
